In order to subscribe to Sponsor Express^© you must complete a subscription request form. This form must be signed by an officer of  the corporation authorizing the data to be sent to the subscriber. This form must then be approved by Slavic's staff. These steps are taken to ensure the confidentiality of the data being transmitted. The subscription request form is a pdf file, which requires the Free Acrobat Reader to view and print. The form may be edited online, but requires printing in order to sign and submit for processing.
