This is for printable Enrollment forms. Form will need to be printed and completed, then faxed or mailed to PlanRight for processing.

Enrollment form processing usually takes 30 days from the date it is received by PlanRight. First, it is checked and entered on the PlanRight system; then it is stamped by PlanRight and sent to your employer for payroll processing. Your employer should not take money out of your paycheck until they receive the PlanRight stamped form. You may make up deferrals from your plan entry date if your form processing is delayed, however, no missed market gains will be reimbursed as this is done on a best efforts basis. Enrollments with improper investment allocations will not be processed. You may only select the funds available on the form and your allocation must total 100%.
